CANCELLATION POLICY:

If you need to cancel or reschedule your appointment, we ask that you please notify the office as soon as possible so that we may free up that spot for another client.

PAYMENT:

Payment is due at the time of service. No refunds are issued.

MEDICAL CONCERNS:

It is imperative that you provide us with your most accurate medical history so that we may serve you better. Any and all questions about procedures or medications will be handled by one of our providers.
